Dr Mallais is a Premier Provider with Delta Dental. We will happily bill any other dental insurance that we are out of network with given you have out of network benefits.

We are not able to bill Medicaid as we are not a Medicaid provider.

Our whole family goes to this office. All the hygienists are extremely friendly. They do a great job cleaning and caring for our teeth. The office staff members are very professional and courteous. Dr. Mallais is an excellent dentist. I would highly recommend this dentist office!
