Dr Mallais is a Premier Provider with Delta Dental. We are participating with Blue Cross please call for more information!
We will happily bill any other dental insurance that we are out of network with given you have out of network benefits.
We are a Healthy Kids Provider, but at this time unable to accept New Familes with this insurance.
We are not able to bill Medicaid as we are not a Medicaid provider.
